Dog Health Questions: Dog training and terrible twos?

im paper trainig my dog because its hard for me to take my dog out often because of a physical disability so he was doing really well with his paper training he is 3 months old almost 4. hes being a big rascal and sometimes peeing on the floor and pooping on the floor (mostly pooping) what is going on? why wont he go where he is supposed to? he is asserting his independace i think (terrible twos?)
